SEIZURE DISORDERS
Meaning of Seizure: Seizure is an abnormal electrical discharge of cerebral neurons accompanied by motors, sensary or affective phenomenon with or without loss of consciousness.
Types of Seizure disorders
There are three types of seizures in children. These are:
(a) Neonatal seizure
(b) Febrile convulsion
(c) Eplipsy
(A) Neonatal Seizure: Neonatal Seizure is seizure in the first twenty-eight (28) days of life.
(B) Febrile convulsion: Febrile convulsion occur between six months and fives years in children.
(C) Epilepsy: Eplipsy occur after five years in children, at times it stays through out the life time of an individual.
Of all the three types of Seizure disorder, only Febrile convulsion Wil be discussed in this page.
FEBRILE CONVULSION
This occurs due to excessive fever as the child's thermoregulatory mechanism as not well developed to prevent it. It is an abnormal response of the developing brain to higher fever.
Clinical Features
(2) Occur in fifteen per on thousand case of high fever within the age group.
(3) Age is between 6 months and 5 years.
(4) Occur more in male than female
(5) there is usually a family history of Febrile convulsion.
(6) incontinence of urine and faeces
(7) post ictal sleep.
(8) it is short lasting, not more than fifteen minutes.
(9) it is a generalized type of Seizure, that is, the tonic and clonic type.
Predisposing Factors
(1) malaria
(2) UTI
(3) otitis media
(4) pneumonia
(5) tonilitis
(6) measles.
Management of Febrile Cconvulsion
(2) take off all the child's clothing.
(3) control fit with either of:
(a) Im paraldehyde 1cc per year of life i.c one year 1ml, 2 years 2ml, etc. Do not exceed 5cc(ml) even if patient is over five years.
(b) IV valium 2-5 mg slowly depending on age.
(4) clear airway: such and extend neck of baby.
(5) tepid sponge and expose to fan if Febrile.
(6) gastric wash out by Ryle's tube if any native drug has been used.
(7) take short history and examine the child properly.
(8) dextrostix = to exclude hypoglycemia.
Blood film = for malaria parasite.
Lumber puncture = to exclude meningitis.
(9) treat based on findings
(10) give IV fluid if child is not taking orally.
